Pathways is an organisation inspired and developed by service users, for service users. Our approach is humanistic and person centred and our aim is to tailor all support plans to the individual and their needs.
We strive to provide a safe and supportive environment in which you can gain the confidence and skills necessary to begin the transition back to independent living.
Following a full assessment of your needs, and in consultation with yourself, you will be allocated a support worker who will be responsible for formulating and agreeing appropriate support aimed at helping you to achieve your goals. This will be reveiwed on a regualr basis as your needs change and will always be done with your involvement and choices at the core of the plan.
There are weekly meetings which you are expected to attend and participate in, as they are an important part of your development. You will also have regular one-to-one sessions with your allocated worker who will be able to offer any individual support you may require. Our support workers provide a 24 hour on-call service for any service users who may be in need of guidance and support.
The association has close links with local housing providers which helps us to provide support and guidance to our service users during the later stages of the transition to independent living.
